I think it's a general comment on how rude that Sherlock was and how the BBC is transphobic by default.
I think that Sherlock, in character, would just insult your intelligence regardless of gender issues.

Most sociopaths are high-functioning, and very good at hiding the fact that they're sociopaths. The fact that Holmes would admit to be a sociopath actually makes him a liar, since concealing your sociopathy is how you function at a high level.
