It's a form of survivorship bias and availability heuristic. All the times a ship goes to a planet and doesn't do anything because of the Prime Directive would make for a terrible episode. We don't get to see every day the Enterprise is just cruising around doing mundane shit.
Then why write in the Prime Directive to begin with?
I think the Prime Directive is misunderstood. It's simply a directive that's wholly divisible only by itself and one.

I don't think the Prime Directive explicitly says you CAN'T have sex with alien women - no matter how primitive their technology is.
As long as you don’t show your alien lover any federation tech you should be fine with any boots knocking. Even if they’re prewarp.
