Currently in mid act 3 and not a hint of romance. Did I mess up? Do you think starting a romance in Act 3 will be added in the future update?

You need to romance her before entering the cursed lands.

Approval needs to be high enough then she will ask to see each other after everyone goes to sleep.

Easiest after goblins/grove interaction during the party, but not necessary. In an evil play though I romanced her in grymforge.

Not necessarily. During my durge run she told my character they'd go skinny dipping once everyone was asleep. Emperor cock blocked my guy though. Never happened.

I had skipped the grove and somehow got enough rep with her to proc that interaction. If you miss the skinny dipping thing though, not possible. Withers is a good way to figure out if you done goofed. He'll tell whomever character approaches him that they got no removed when you have no active romance options, and that they should care for them if they do.

I didn't get the chance to, so I guess I have to edit my save file:/

Why? Seems like a lot of effort for a whole lot of nothing. Why not just allow the story to play out as is. The game reflects your choices.

I don't want to do a whole other playthrough to see that content and it really fits with the narrative of my character that I have in mind. Other wise I wouldn't care.

Have you had any romantic interactions with her at all in any act? If not, I doubt you’re getting frisky with her starting in act 3, but I could be wrong.

I told her I'd look for night orchids and also made a move at the Grove celebration but I didn't have enough approval. I assumed there would be more opportunities. I wish the game would tell you that it's necessary in act 1 because from what I've seen a lot of people missed it.

You can manually trigger the lake scene with a scripting mod (u/Tiki_Man_Roaron on Reddit figured this out):

  • Install Script Extender with Console (manually or from Nexusmods)
  • Start game, it should launch a text console in another window
  • Load savegame, travel to Rivington
  • Focus console, press enter, insert the following command and confirm with enter again:
    • Osi.PROC_CAMPDEBUG_QueueNight(“NIGHT_Shadowheart_Skinnydipping_9f583304-0a1a-498c-acf9-3c8dcc30ee3d”, 1)
  • Start long rest in Rivington

You should see the exclamation mark above her head, and the scene should play after talking to her and going to sleep. If you want you can remove the Script Extender mod afterwards.

I've already found a thread about how to activate her romance with cheat engine and stuff. Would activating this scene start a romance with her or would it literally just be the scene?

For me it just played the scene and triggered the dialogs before and after. But I already had a romance with her, so idk what happens if you don't have that.
