It takes $5.8 million to be part of the 1% in the USA. Do many engineers or doctors have that much?

In Germany at least only if they have their own company and exploit their employees (or inheried wealth), but Germany doesn't treat it's engineers very well.

Our of interest is that shared income or personal? I wonder how many people are part of the 1% but their spouse and children aren't.

My source doesn't explicitly mention it. But underage children normally don't count in lists like this.
