Have you tried replacing the laser? Not reading would mostly be that. The grinding would most likely be the spindle? I'm not sure on that one.
Grinding/scratching is often actually the lens, blocked by dust, trying to focus on the disc but failing to focus and adjusting too high and scratching the disc, only making matters worse.
