That's because XCOM's accuracy percentage is not exactly a "chance to hit". It's a modifier that determines whether the shot is going to go wildly off-target or not, and by how many degrees.
A 100% accurate shot is always going to use the more accurate cone-of-fire, but it may be as much as 1-2 degrees off, which would matter for longer shots.
And that's why I prefer Phoenix Point's targeting system

Until you get an effect on every joker. Then it will deny you the ability to even hit the button.

Wdym? That is exactly how two Oops All Sixes behaves and would 100% proc wheel of fortunes. It will also break all glass you played and space joker will always level your hand up
I know, I was just being mad at the wheel, lol. That's a fun combo if you can get mults and some chips.
