They sign up for it because the advertising is insidious. My family member signed up for it because it used the name of a well-known doctor's group in the area. Sure, they have a deal with said doctor's group, but it is no better than having the entire field available that medicare would have given.

Aye, anecdotally, there was a school near my childhood home, other side of the street and about .8 miles away. The line of cars used to be about halfway to my house. Now, when I visit family, I see it stretching past my house and it at least stretches a full mile in length now. It's nuts.
It's not quite as mandatory as it can get. The FCC has made rumbles about actually kicking carriers off the networks if they don't adhere to the shaken/stirred protocols, but never seems to get around to actually doing it.
