Honestly at this point it's mostly about doing a root cause analysis to find out what specifically caused this tragedy. Unfortunately there are a lot of possibilities for what could have gone wrong. The more that comes to light about this whole thing the more obvious it becomes that it wasn't a question of if this tragedy would happen, but when.

Going by how that sub was mcgyvered together and testimonials of previous passengers saying they always lost contact, I'd say that a lot is an understatement
