Seems like a rather expensive mistake to make.
Wonder if it was a design issue, or operator error.
I think the tanks are pressurized using some sort of compressor while at the rocket garden. Maybe one of those compressors / hoses etc failed? Could be due to any number of reasons not directly related to the ship itself, but as you say, expensive mistake.
