After a year of not having this problem its back: just about every time I get in the car it starts playing spotify, even if I hit pause (or whatever the left scroll wheel press is called) before I get out.

I swear it switched from bluetooth audio back to spotify on its own, but I can't prove that.

I have been using spotify on my office desktop (on the same account), does any use of spotify on any device perhaps trigger always playing in the car?

Is there an iron clad way to keep it from starting playback every time I get in?

I’ve noticed the same thing. Maybe experiment with stopping it before putting the car into park, as it switches to the easy entry profile. I haven’t found a 100% fix. It occurs to me that I always hear it from outside the car when I use summon mode, I wonder if that might be when it gets restarted.
