What is the source for this? Was this made up from the S08E18 ("Threads"), or was this released by the production somehow?

I don't remember how I procured this image, but I got it with the same stuff that included all those official production drawings.

That's a wayback machine capture of the site marked in the corner. Comparing to a screengrab in the episode, this does seem like a recreation rather than the actual copy of the prop.
