Was Big Boss actually in the game? It's supposed to be him in the opening with the bandage? Does he reappear for real?

Yes he is in the game. He is Ahab. He also appears in the end. But the guy YOU play as is not the real Big Boss.

In a literal sense of the plot of the game, totally correct. I'd just like to add that in a thematic sense Big Boss is in the entire game because you the player are Big Boss, we are all Big Boss.
